  • No provider–patient relationship is created through your use of this site, submission of a form, or booking a consultation. Clinical evaluation and treatment can only occur in person with a licensed provider.

  • Pricing, financing, eligibility, and timelines are subject to change without notice.
  • Any offers, promotions, or financing are contingent on credit approval and internal underwriting.

3. Warranty, Maintenance & Limitations

  • If we offer any warranty on implants, it will be strictly governed by conditions (e.g., follow-up visits, hygiene, treatment at authorized locations).
  • Warranty is void if the patient fails to follow our post-care instructions, uses unauthorized providers, or if alteration or neglect contributes to deterioration.
  • We are not responsible for complications due to external factors (e.g., trauma, systemic disease, abstaining from care, or noncompliance).

4. User-Generated Content & Reviews

  • By posting content (reviews, images, messages), you grant us a non-exclusive, perpetual, royalty-free license to use it for marketing, educational, or internal purposes.
  • You must own or have rights to any content you post.
  • We reserve the right to remove or reject any content.
  • You acknowledge that by posting publicly, some health details may no longer be protected under PHI rules.

5. Third-Party Links & Services

Our site may contain links to third-party sites or embed third-party tools (e.g., calendars, chat, financing). We are not responsible for third-party content, policies, or actions. You use such links and tools at your own risk. Each third-party service is governed by its own terms and privacy policies.

California Civil Code §1542 Waiver

You waive the provisions of California Civil Code §1542, which states:

"A general release does not extend to claims that the creditor or releasing party does not know or suspect to exist in his or her favor at the time of executing the release and that, if known by him or her, would have materially affected his or her settlement with the debtor or released party."

This waiver means you release us from unknown claims related to third-party content or interactions.

Costs & Frequency: Message and data rates may apply. Frequency varies by interaction; typical 1–8 msgs/month for marketing; appointment flows may include additional confirmations and reminders.

Privacy: Personal data, including your mobile number and message content, is handled per our Privacy Policy. We do not sell mobile numbers. We share with service providers strictly to deliver messaging.

8. Accuracy & Security Disclaimer

  • We make reasonable efforts to keep content accurate and up to date but make no guarantees of accuracy, reliability, or completeness.
  • We do not guarantee that the website will be free of errors, viruses, or other harmful components. You are responsible for implementing adequate security measures.
  • We are not liable for damage to devices, data loss, or security incidents arising from use of the site.

9. Disclaimers & Limitation of Liability

11. Age & Eligibility

  • You represent and warrant that you are at least 18 years old or have parental/guardian consent if younger. You have full authority to agree to these Terms.

12. Governing Law & Dispute Resolution

These Terms are governed by the laws of the State of California. Any dispute shall be brought in the state or federal courts located in Placer County, California.

Binding Arbitration and Class Action Waiver:

At our discretion, disputes may be resolved through binding arbitration conducted in California under AAA rules. You waive any right to a jury trial and to participate in class actions, to the maximum extent permitted by law.

If a provision is found invalid or unenforceable, the remainder shall still be enforced to the fullest extent permitted.

13. Severability & Survival

  • If any part of these Terms is invalid, the rest remains intact.
  • Sections regarding intellectual property, disclaimers, indemnification, warranties, limitations of liability, and dispute resolution survive termination or expiration of these Terms.

15. Price Match Guarantee

Fusion Dental Implants Club may, at its sole discretion, offer a Price Match Guarantee on qualifying treatment plans. To be eligible:

  • The competing offer must be in writing, presented to our office within 7 calendar days of the date on the competitor's offer, and must be current and valid at the time of review.
  • The offer must come from a licensed dental provider within the same regional geographic area as our practice. National chains, traveling implant teams, overseas providers, and offers outside this region do not qualify.
  • The competing offer must be for the exact same services, materials, treatment scope, and quality standards as our proposed treatment. Any differences in procedure type, number of implants, materials, laboratory work, follow-up care, or warranty terms may disqualify the offer.
  • The competing provider must meet equivalent qualifications, facility standards, and follow-up care protocols. Offers from lower-tier providers, discount clinics, or clinics not providing full-service treatment (e.g., no on-site surgical or restorative capabilities) are not eligible.
  • We reserve the right to independently verify the offer, including contacting the provider.
  • Fusion Dental Implants Club retains absolute discretion to reject any price match request that, in our judgment, does not represent a legitimate, comparable offer or fair practice.
  • Price match applies only to the cost of the identical treatment services; it does not extend to ancillary offers, financing terms, promotional discounts, or bundled services outside the treatment scope.
  • This Price Match Guarantee is a courtesy, not a contractual obligation, and may be modified or discontinued at any time without prior notice.
